My Ls's barrel keeps coming out. It is CPVC.
The bolt has been cut down to about where the o-ring is in this picture.

I'm assuming you're referring to a singled LS's bolt, as the barrel wouldn't be connected to that piece. It's probably your choice of adhesive. Use something like epoxy for this.
